Monument Health wants to keep all of our area athletes going higher, faster and farther. To prep our athletes and provide the best care to our communities, we’re offering free sports physicals.
Spots are limited. Registration is required. Please call 605-755-CARE (2273) to register.
All student athletes must be accompanied by their parent or legal guardian. If your student has not been able to play sports due to a medical condition, please see your primary care physician to complete your sports physical requirements.
