Application Requirements

• Applications must be submitted at least two months prior to the start date of the program or event.
• Request must include all programs and events for which funding is being requested for the entire calendar year.

What We Do Not Fund

• Advertising
• Food and beverages
• Swag items or prizes
• Requests for individuals
• Political events or activities
• Personal objectives of Monument Health physicians or caregivers (e.g., a caregiver’s child’s sports team)
• Requests outside Monument Health’s service area
  (Note: Monument Health’s service area includes the following counties in western South Dakota: Pennington, Lawrence, Meade, Butte, Custer, Fall River, and Oglala Lakota.)

Review Process
All requests are reviewed based on alignment with our priorities and the availability of funds. Monument Health considers all applications without discrimination based on race, color, religion or age.

Community Health Screenings: Monument Health is committed to impacting our communities. It is one of our top priorities. Our goal this year is to provide over 20,000 health screenings to our friends and families in the Black Hills. The focus is to provide preventive care, close to home.

Sports Medicine Coverage: Our Sports Medicine practitioners and certified athletic trainers are Monument Health’s front line to athletes at every level. We work closely with our therapists and physicians in communities throughout the Black Hills.

Special Rodeo:  Twice a year Monument Health puts on a Special Rodeo for individuals in our community with disabilities.
