COMMUNITY HEALTH NEEDS
Every three years, Monument Health conducts a Community Health Needs Assessment (CHNA) for its hospital communities. Areas in these assessments include Butte, Custer, Fall River, Lawrence, Meade, Pennington and Oglala Lakota counties in South Dakota and Crook County in Wyoming.
CHNA is a systematic, data-driven approach to determining the health status, behaviors and needs of residents in the Monument Health service area. Based on the needs identified in the CHNA, Monument Health also compiles a three-year Community Health Implementation Plan.

- July 15
- October 15
Monument Health’s Priorities are:

- Civic and/or Community Development
- Sports Medicine
Monument Health DOES NOT provide sponsorship funding for the following:
- Advertising
- Food/Drinks
- Swag Items/Prizes
- Individuals
- Religious or political groups or events/activities
- Monument Health physician and/or caregiver personal objectives (i.e. supporting a specific caregiver’s child’s sporting event or a program that does not meet the overall community or regional focus)
Requests serving residents of Monument Health’s service area will be given priority. The Monument Health service area includes the following counties in western South Dakota and eastern Wyoming: Pennington, Lawrence, Meade, Butte, Custer, Fall River, Oglala Lakota, Crook and Weston.
Requests are reviewed within the context of available funds. Monument Health considers all applications without discrimination based on race, color, religion or age.
COMMUNITY ENGAGEMENT
Community Health Screenings: Monument Health is committed to impacting our communities. It is one of our top priorities. Our goal this year is to provide over 20,000 health screenings to our friends and families in the Black Hills. The focus is to provide preventive care, close to home.
Sports Medicine Coverage: Our Sports Medicine practitioners and certified athletic trainers are Monument Health’s front line to athletes at every level. We work closely with our therapists and physicians in communities throughout the Black Hills.

Special Rodeo: Twice a year Monument Health puts on a Special Rodeo for individuals in our community with disabilities.
