It is our privilege to support you in obtaining health information for continuing health care, billing, insurance or administrative purposes.To begin the process, please complete the Authorization for Disclosure/Release of Protected Health Information form. You may print, complete and mail or hand deliver the form to one of the locations below or email it to RHHIMReleaseofinfo@monument.health.
Effective July 1, 2023, a standard fee schedule will be followed to produce records/health information, including allowable fees as identified by federal, state, or local law or regulation. Please review the information fee schedule here.
You can also use MyChart, our unified patient portal, which gives you secure online access to your health care records. You will need to sign up or login to your account, click ‘Health’, click ‘Document and Record Center’, click ‘Your Medical Records’, fill out your release electronically, and the records will be released to your MyChart account soon thereafter.
How do I request a copy of my records?

Who can sign the authorization form?
You or a representative who is on your HIPAA or Power of attorney paperwork.
How much does it cost to get medical records?

How do I get medical records for a deceased patient?
The estates representative or Power of Attorney on file may request.
How long does it take to get my records?
HIPAA Privacy Rule indicates providers have 30 days to deliver records from the date of your request. However, Rapid City Medical Center strives to complete record request in a timely manner. Please allow a minimum of 3-5 days for processing.
