DOT PHYSICALS

Monument Health offers DOT physicals for the convenience of area CDL holders. This exam includes a detailed medical history and physical exam that tests vision, hearing, blood pressure, blood sugar, drug use and other health indicators. The cost is $100.

Medical Examiner's Certificate
