EpicCare Link is a secure web portal that connects you to information stored in our electronic health record (EHR) for your referred and admitted patients. EpicCare Link offers view-only access to patient records. When your patients receive care from Monument Health, we’ll send you notifications and updates through EpicCare Link. You’ll have the information you need to plan patients’ ongoing care and they’ll feel better, knowing that you’re up to speed on all the care they receive.

Continuity of Care
Community providers can securely access medical records, which can improve the continuity of quality care for the patient. When you refer patients to Monument Health via EpicCare link, you can track the process from start to finish and view your patients’ records after they are seen by one of our providers.
We’ll keep you informed about their care. With automatic messages about important events, you can have the latest information about your patients’ treatment. We’ll notify you to log in to see:
- Lab, imaging and test results
- Hospital admissions
- Discharge summaries
- PACS images
- Consultation notes
- Orders placed by our physicians
Patient Outreach
Refer patients from your browser. You can request consults, schedule appointments and send messages to our providers directly from your browser. You no longer need phone or fax – we’ll send you confirmations and patient updates in real time.
Review billing documentation. Your billing staff can view clinical documentation for admitted patients to verify the documentation supports the charges.
Receive HIM information. EpicCare Link integrates with Epic’s Health Information Management which allows us to release standard Request of Information reports to your organization and other requestors.
Reporting
Community users can view standard reports within EpicCare Link, which will allow you to track and follow up on trends, such as your referrals.
