RAPID CITY, S.D. (Aug. 31, 2018) – Smiles, orange juice and bagels were the theme during a Friday morning event on the fourth floor of Monument Health Rapid City Hospital. As of Sept. 1, dedicated Monument Health physicians and community based physicians will be partnering with Monument Health Home+ to extend medical care to patients who live in long-term care facilities and nursing homes throughout the Black Hills.
This program allows patients to stay at their current living situation; the patient will not have to leave the nursing home to see a physician. The doctor comes to the patient. This will allow for better care, a more stable living arrangement and more comfort for patients when being treated by a doctor. The newly formed dedicated team will provide consistency to the patients across the Black Hills through the partnership of Monument Health Home+ and the physician group.

Monument Health will serve Bella Vista Care and Rehabilitation Center, Prairie Hills and Rehabilitation Center, Fountain Springs Healthcare Community, Good Samaritan St. Martin’s Village, Black Hills Care and Rehabilitation, Golden Living Center Meadowbrook, and several other long-term care facilities and nursing homes. “The ability to see patients in their current living situation will elevate nursing home patient care in our region,” said Mike Tilles, President of Monument Health Home+. “Our goal is to ensure every patient has the right care at the right time, and to make a positive impact in their lives. If there is anything we can do to improve care and make things easier for patients, we want to do it. The goal is to grow throughout the Black Hills and partner with facilities and doctors to provide the right care, at the right time, at the right location for all patients.”
