Transitional Care

Monument Health’s Transitional Care Program provides skilled nursing care and rehabilitation services for patients who are discharging from the hospital but still need temporary assistance that cannot be provided at home or in a typical skilled nursing facility. The program is designed to assist the patient in becoming as independent as possible before discharge.

Once able to safely discharge, they work with the patient’s primary care provider to coordinate any in-home services needed.

To qualify for the Transitional Care Program, the following requirements must be met:

  • Have had three inpatient overnights in an acute care bed prior to the Transitional Care Program.
  • Have an ongoing diagnosis and needs that require skilled care as defined by Medicare.

Shockwave Therapy

Shockwave therapy is a technique used in orthopedics, physical and occupational therapy, and sports medicine. It is a non-surgical, non-invasive therapy that transmits focused acoustic waves to the affected tissue. These waves stimulate the body’s natural healing mechanisms to aid in the reduction of pain, restoration of normal tissue function, and reducing the need for medications and/or significant activity modification.

Deep Tissue Laser Therapy

Laser therapy is used in orthopedics, physical and occupational therapy, and sports medicine. It is a non-surgical, non-invasive therapy that uses focused light to stimulate a process called photobiomodulation (PBM). During PBM, photons enter the affected tissue and trigger a biological cascade of events that leads to an increase in cellular metabolism, which can decrease pain and inflammation and accelerate the healing process.

Occupational therapists help patients with skills for living, such as caring for themselves, cooking, homemaking and working. We work with people of all ages who, because of illness or injury, need to re-learn skills to return to productive, independent and satisfying lives.

Counseling

Licensed Clinical Social Workers (CSW) in Rapid City, Lead-Deadwood and Sturgis provide outpatient assessment and treatment in the medical clinic setting.

Social Workers collaborate with medical doctors, psychologists and psychotherapists as deemed necessary and often work in consultation with other service agencies such as schools, health providers, rehabilitation programs, courts and alcohol drug programs is. Inpatient psychiatric care and facilities are available if deemed appropriate.

Emergency Medicine

Monument Health Hospitals in Custer, Lead-Deadwood, Rapid City, Spearfish and Sturgis provide 24/7 emergency medical services. 

All Emergency Departments treat injuries, illnesses that are immediately life-threatening and other emergent conditions of both adult and pediatric patients. Teams of doctors, registered nurses, advanced providers and other specially trained staff are available 24 hours a day, seven days a week to treat anyone seeking emergency medical care. Each year, Monument Health providers see more than 75,000 patients in the emergency care setting.


Rapid City Emergency Department

In addition to treating more common urgent or emergent medical conditions, the Department of Emergency Medicine at Rapid City provides Level 2 Trauma services, caring for patients with extremely severe injuries and serves as a referral center for people with traumatic injuries throughout Western South Dakota, Western Nebraska, Western North Dakota, Eastern Wyoming and Eastern Montana.  

As the region’s only accredited stroke center, Rapid City Hospital’s award-winning Interventional Neurologists on the Stroke Care team are the area’s recognized leaders. The Heart and Vascular Institute is also the largest provider of full scale cardiovascular services in western South Dakota. These nationally-lauded services collaborate closely with the Emergency Department and are often the difference between life and death in emergent heart failure and stroke victims.

Emergency Services & Medical Campus

On June 10, 2026, the Emergency Services & Medical Campus at 1911 East Mall Drive opened to meet the community’s growing need for emergency care. Just off I-90 at Exit 60, across from the Toyota dealership, the standalone emergency facility is more convenient for the growing Rapid City community along the I-90 corridor.

Doctors from the Emergency Department at the Rapid City Hospital will be delivering the same high-quality care and caring experience at this new location — specializing in Emergency Medicine, Lab On-Demand, Laboratory Services and Medical Imaging. It is open 24 hours, 365 days a year for all.

Sturgis Emergency Department

The Sturgis Emergency Department is state-certified as a South Dakota Department of Health trauma receiving hospital and a Centers for Medicare & Medicaid Services Critical Access Hospital (CAH).

Sturgis Emergency Department is a proud participant in the American Heart Association’s Mission: Lifeline acute stroke and post-acute stroke projects. Our participation in the Mission: Lifeline Stroke initiative demonstrates our commitment to quality care. We are proud to be a part of the American Heart Association’s efforts to turn guidelines into lifelines.

In addition, the Sturgis Emergency Department provides access to other providers who specialize in emergency care via Telehealth. 

Spearfish Emergency Department

The Spearfish Emergency Department is a South Dakota Department of Health Designated Community Trauma Hospital (CTH) open 24/7 and is fully equipped to care for patients of all ages in critical and urgent situations. With trauma-trained nurses and providers ready upon patient arrival, Spearfish ED evaluates, stabilizes and treats trauma patients with immediate ATLS-compliant care protocols and surgical resuscitation procedures prior to transfer (if required). We are also equipped with 24/7 laboratory services, medical imaging and respiratory therapy.

Backed by a Spearfish Hospital’s full range of services, including: Inpatient care, Labor and delivery, Intensive care and Surgical specialties, Spearfish ED is Staffed with experienced emergency medicine providers, including physicians and advanced practice providers, with increased staffing during high-volume midday hours. Immediate access to on-call specialists in General Surgery, Orthopedic Surgery and Obstetrics and Ear, Nose and Throat, ensures timely expert evaluation and intervention.

Lead-Deadwood Emergency Department

The Lead-Deadwood Emergency Department is state-certified as a South Dakota Department of Health trauma receiving hospital and a Centers for Medicare & Medicaid Services Critical Access Hospital.

Custer Emergency Department

The Custer Emergency Department is state-certified as a South Dakota Department of Health trauma receiving hospital and a Centers for Medicare & Medicaid Services Critical Access Hospital. Open 24/7, Custer ED offers outpatient infusion services for medication injections and infusions, 12-bed inpatient services, respiratory therapy, 24/7 Lab and Radiology and a doctor available 24/7. 

Leading Rural Health Care

Critical Access Hospitals

The Sturgis, Lead-Deadwood and Custer Emergency Departments are Critical Access Hospitals. CAH is a designation designed to improve access to healthcare by keeping essential services in rural communities. Eligible hospitals must be more than 35-miles away from the nearest hospital/CAH, maintain 25 inpatient or swing-beds and maintain 24-hour emergency care services 7 days a week.

Designated Trauma Hospitals

The Sturgis, Lead-Deadwood and Custer Emergency Departments are Trauma Receiving Hospitals. South Dakota divides trauma hospitals into five groups: regional, area, community, receiving and non-trauma. A trauma receiving hospital must maintain 24/7 emergency department coverage by physician, physician assistant or nurse practitioner.

Community Trauma Hospital (CTH)

As a CTH, Spearfish Emergency offers 24/7 physician coverage and surgical coverage for all trauma team activations, with trauma transfers for when surgery is not available.

All emergency staff maintain certifications in:
• Advanced Trauma Life Support (ATLS) through the American College of Surgeons
• Trauma Nursing Core Course (TNCC) through the Emergency Nurses Association
• Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) through the American Heart Association
• Additional emergency care competencies and ongoing training
