Palliative Care
Palliative Care can be provided in many settings including hospitals, clinics and in the home. Our team consists of providers, social workers and registered nurses. Spiritual services are also available. Palliative Care can decrease suffering, worry and anxiety related to serious illness while increasing the sense of support. It can also provide clear direction for next steps once the hospital stay is concluded.
Eligibility for Palliative Care
Palliative care is appropriate for anyone suffering a serious, chronic or life-threatening illness. Our providers assist with goals of care conversations and provide symptom management. Ask your physician for a consultation with our palliative care team or contact the Monument Health Palliative Care provider in your area.
