LACTATION SERVICES

Lactation Services in Rapid City and Spearfish consist of registered nurses who are International Board Certified Lactation Consultants (IBCLC). Lactation consultants provide education and support for parents who choose to breastfeed their infant(s).

Achieving IBCLC status requires specialty education and extensive practical experience working with breastfeeding mothers before taking and successfully passing a university-level certification exam. Monument Health’s standard of providing lactation consultants with IBCLC status assures you will have the highest level of expertise available.

BREASTFEEDING BENEFITS

The American Academy of Pediatrics stated in its 1997 policy statement on breastfeeding that “Human milk is uniquely superior for infant feeding and is species-specific; all substitute feeding options differ markedly from it. The breastfed infant is the reference or normative model against which all alternative feeding methods must be measured with regard to growth, health, development and all other short- and long-term outcomes.”

Hip Preservation Institute

Hip preservation is a field of orthopedics focusing on treating hip pain in adolescents and active young adults to prevent the development of early arthritis. Hip preservationists use special X-ray views, advanced MRI, low-dose CT and 3D reconstructions to identify abnormal hip mechanics that can cause pain, cartilage damage and labral tears. A combination of arthroscopic and open hip procedures are used to address the abnormal anatomy and damaged cartilage.

ROSA® Knee Robotic Technology

You’re unique, and so is your individual anatomy. ROSA®, which stands for Robotic Surgical Assistant, is designed to help your specially trained surgeon tailor the placement of your knee implant just for you.

Getting a precise knee implant fit is important to your comfort and overall experience following knee replacement surgery. ROSA® Knee uses data collected before and during surgery to inform your surgeon of many details related to your unique anatomy that may affect your implant fit. By using this data to make more informed decisions, your surgeon is able to plan for and carry out a personalized surgery based upon your individual needs.

ARTHRITIS IN THE KNEE JOINT

The knee is a hinge joint formed by the tibia (shinbone), femur (thighbone) and patella (kneecap). The ends of the bones in the knee joint are covered with cartilage, a tough, lubricating tissue that helps cushion the bones during movement.

Osteoarthritis, the most common form of arthritis, is a wear-and-tear condition that destroys joint cartilage and bone. It typically develops after years of constant motion and pressure in the joints. As the cartilage continues to wear away, the joint becomes increasingly painful and difficult to move. If conservative treatment options fail to provide relief, your surgeon may recommend total knee replacement using ROSA® Knee robotic technology.

BEFORE SURGERY

Your preoperative experience will be like that of most total knee patients. But, unlike traditional knee replacement methods, with ROSA® Knee, a series of x-rays may be used to create a three-dimensional (3D) model of your knee anatomy. This 3D model will enable the surgeon to plan many specifics of your knee replacement prior to your surgery.

DURING SURGERY

The surgical procedure using ROSA® Knee is similar to traditional total knee replacement, but with a robotic assistant. Your surgeon has been specially trained to use ROSA® Knee in order to personalize the surgical approach for your unique anatomy. It’s important to understand that the robot does not operate on its own. That means it does not move unless your surgeon prompts it to. Your surgeon will make intraoperative decisions based on data points and stress range of motion to position your implant in the optimal position. ROSA® then positions the cutting guides to allow the surgeon to make precise cuts for optimal placement of your implant. Your surgeon is still in the operating room the entire time and is making all of the decisions throughout your surgery.

During your procedure, ROSA® Knee utilizes a camera and optical trackers attached to your leg to know exactly where your knee is in space. Think of it like a very detailed global positioning system (GPS) that you might use in your car. If your leg moves even a fraction of an inch, the robot can tell and adjusts accordingly. This helps ensure that the plan your surgeon put into place is executed as intended. Throughout your surgery, ROSA® Knee provides your surgeon with data about your knee. This information, combined with your surgeon’s skill, helps them know how to position your implant based on your unique anatomy.

AFTER SURGERY

Following surgery, you will be hospitalized based upon the recovery plan your surgeon decides is best for you. This hospitalization may range from one to three days. Recovery time varies, but most people should be able to drive after two weeks, garden after three to four weeks and golf after six to eight weeks. Your surgeon will tell you when and what activities you can return to, and what activities to avoid.

Monument Health currently has a bronze level certification as a Safe Sleep Hospital from Cribs for Kids. The Cribs for Kids National Infant Safe Sleep Hospital Certification Program recognizes hospitals and hospital systems for their commitment to implement American Academy of Pediatrics evidence-based practices to reduce the rate of unsafe sleep injuries and Sudden Unexpected Infant Death (SUID) related to unsafe sleep, including Accidental Suffocation and Strangulation in Bed (ASSB) and SIDS.

SUID and Safe Sleep Overview

Sudden Unexpected Infant Death (SUID), which includes Sudden Infant Death Syndrome (SIDS), is the leading cause of injury death in infancy. Sleep is a big challenge for families with babies, but following safe sleep recommendations can prevent many SUID fatalities. Help the families in your practice learn how their babies can sleep safely so parents can sleep soundly.

The rate of sleep-related infant death declined significantly in the 1990s after the AAP and others recommended that babies be placed on their backs to sleep, but rates have since plateaued, and SIDS remains the leading cause of post neonatal mortality. Black and Native American/Alaska Native infants die at rates more than double that of white babies.

South Dakota Infant Mortality Rates

While South Dakota saw dramatic improvements from the 1960s to the 1990s, progress has stalled in recent years. The state’s rates remain consistently higher than the national average, with preventable factors such as congenital malformations, unsafe sleep conditions, and maternal health risks contributing to deaths.

Sleep-related infant deaths are a serious concern. Unsafe sleep environments put babies at higher risk for tragedy—and many of these deaths can be prevented

Between 2019 and 2023, 89 sleep-related infant deaths were reviewed by the South Dakota Child Death Review team.
Sudden Unexpected Infant Death (SUID) accounts for nearly 1 in 5 infant deaths in South Dakota, with unsafe sleep conditions being a major contributing factor.
63% of these sleep-related infant deaths occurred while baby was sharing a sleep surface with an adult and/or child.
Unsafe sleep environments include adult beds, couches, chairs, car seats, swings, rock ‘n plays, or cribs that do not meet safety guidelines.
87% of these sleep-related infant deaths were considered potentially preventable after review.

What We've Learned from 10 Years of Infant Mortality Data

South Dakota’s infant mortality rate has remained stable over the past decade despite national declines.

Congenital malformations have remained the leading cause of infant mortality (~25% of deaths).

Sudden Unexpected Infant Deaths (SUID) account for nearly 1 in 5 infant deaths, with unsafe sleep conditions as a major contributing factor.

A growing percentage of sleep-related infant deaths occur in adult beds, increasing from 47% (2014–2018) to 63% (2019–2023).

Maternal smoking, substance use disorder, and maternal health conditions continue to be significant risk factors.

How South Dakota Compares to the U.S.

As the graph demonstrates, except for 2016, our rates persistently exceeded the Healthy People target of 5.0 infant deaths per 1,000 live births. This persistent gap underscores the urgent need for targeted interventions and a renewed focus on addressing the underlying factors contributing to these elevated rates.
