Effective July 1, 2023 A standard fee schedule will be followed to produce records/health information, including allowable fees as identified by federal, state, or local law or regulation. Please review the information fee schedule here.
You can also use MyChart, our unified patient portal, which gives you secure online access to your health care records. You will need to sign up or login to your account, click ‘Health’, click ‘Document and Record Center’, click ‘Your Medical Records’, fill out your release electronically, and the records will be released to your MyChart account soon thereafter.
