All patient appointments can be made by phone. You can also make or request an appointment using the MyChart patient portal and app. Walk-ins are welcome at Monument Health Urgent Care facilities. For emergencies, please call 911.
Monument Health Hospitals
In most cases, appointments for tests, treatments and therapy at Monument Health hospitals will be made by your provider. If for some reason you cannot keep your appointment, or it is necessary to change the date or time, please contact one of the appointment centers listed below. In some cases, you might have to contact your health care provider to reschedule an appointment. Our Patient Appointment Center staff will direct you as necessary.
