Your family doctor knows you best and serves as your health care navigator. Monument Health’s Primary Care Providers (PCPs) establish a baseline of well-being for you and your family. They guide you on your wellness journey, identify risk factors and coordinate with specialists when illnesses or injuries arise.
Primary Care Providers specialize in internal medicine or family medicine. Internal medicine focuses on preventing and treating adult diseases, while family medicine focuses on caring for individuals of all ages.
Our goal is to be your first point of contact and deliver the care that’s right for you.
Four reasons to establish a Primary Care Provider:
- Preventative Health Care
Primary Care Providers provide opportunities for disease prevention and health promotion, as well as early detection of problems. They work to identify health care concerns before they turn into serious health threats, including the management of hereditary chronic illnesses. - Medical Home
Patients can bring a wide range of health problems to their Primary Care Provider for appropriate attention—a place in which patients can expect, in most instances, that their problems will be resolved without referral. - Continuity of Care
Primary Care Providers facilitate an ongoing relationship between their patients and dramatically increase the likelihood that you’ll receive a correct diagnosis and appropriate treatment. By being able to track your health over time, your Primary Care Provider can gain valuable information and provide the best health care for you. - Coordination of Care
Your family doctor knows you best – and serves as your health care navigator. Primary Care Providers establish a baseline of well-being for you and your family. They guide you on your wellness journey, identify risk factors, and coordinate with specialists when illnesses or injuries arise.

What People Are Saying
Dr. Gale is the absolute best primary care physician I’ve ever had. He treats me like a person, not a number – and has a real-life discussion with me about any and all of my concerns and questions. I have a complicated medical history, and he considers all aspects whenever thinking about treatment, medication changes, and referrals. I’m so lucky to have found him.
Dr. Bogard always provides excellent care. His listening and interaction skills with my daughter and myself go above and beyond of what I would expect. He made sure we understood all treatment options and diagnosis before we left for the day. Very attentive, detail-oriented, and caring!
Dr. Williamson was awesome. She asked a lot of questions and provided insight into health risks for my age and gender. I will definitely see her next time I need a doctor.
Dr Falkenburg is so very caring and compassionate. She made me feel like I was the most important patient in the world. She explained, educated and made plans in a very short time and followed through with a referral. Custer is very fortunate to have the highest quality medical professionals.
