Your family doctor knows you best and serves as your health care navigator. Monument Health’s Primary Care Providers (PCPs) establish a baseline of well-being for you and your family. They guide you on your wellness journey, identify risk factors and coordinate with specialists when illnesses or injuries arise.

Primary Care Providers specialize in internal medicine or family medicine. Internal medicine focuses on preventing and treating adult diseases, while family medicine focuses on caring for individuals of all ages.

Our goal is to be your first point of contact and deliver the care that’s right for you.

 

Four reasons to establish a Primary Care Provider:

  1. Preventative Health Care
    Primary Care Providers provide opportunities for disease prevention and health promotion, as well as early detection of problems. They work to identify health care concerns before they turn into serious health threats, including the management of hereditary chronic illnesses.
  2. Medical Home
    Patients can bring a wide range of health problems to their Primary Care Provider for appropriate attention—a place in which patients can expect, in most instances, that their problems will be resolved without referral.
  3. Continuity of Care
    Primary Care Providers facilitate an ongoing relationship between their patients and dramatically increase the likelihood that you’ll receive a correct diagnosis and appropriate treatment.
