Monument Health Behavioral Health Center provides comprehensive outpatient mental health services for children, adolescents, adults, and older adults through a multidisciplinary team of psychiatrists, advanced practice providers and Clinical Social Workers (CSWs). Our team is committed to providing compassionate, personalized care to help individuals improve their mental health and overall well-being.
Our providers evaluate and treat a wide range of behavioral health conditions, including anxiety disorders, postpartum depression, depressive & mood disorders, ADHD, bipolar disorder, PTSD and more.
Our psychiatrists and Advanced Practice Providers (APPs) deliver compassionate, evidence-based psychiatric care tailored to the unique needs of children, adolescents and adults. Services include:
Accurate Diagnosis
Careful evaluation and diagnosis of a wide range of mental health conditions using evidence-based clinical practices to ensure appropriate and effective treatment.
Care Coordination
Collaboration with primary care providers, therapists, schools, specialists and other healthcare professionals to promote seamless, integrated care and improve treatment outcomes.
Comprehensive Psychiatric Evaluations
Thorough assessments that review mental health history, current symptoms, medical background, family history, lifestyle factors and treatment goals to develop a clear understanding of each patient’s needs.

Medication Management & Monitoring
Management: Personalized prescribing and adjustment of psychiatric medications to help manage symptoms while considering each patient’s health history, treatment response and individual preferences.
Monitoring: Ongoing follow-up appointments to evaluate medication effectiveness, monitor for side effects, make dosage adjustments when needed, and support long-term treatment success.
Ongoing Follow-Up Care
Regular appointments to monitor progress, reassess symptoms, update treatment plans, and provide continuous support throughout the patient’s mental health journey.
Outpatient Counseling
Our outpatient counseling services provide compassionate, personalized care while allowing you to continue your daily routine. Treatment may include assessment, diagnosis when appropriate, individualized treatment planning, individual or family therapy, skill-building and ongoing support tailored to your unique needs.
Patient and Family Education
Guidance on mental health conditions, treatment options, medications, expected outcomes and self-management strategies to empower patients and their families to make informed decisions.
Referrals to Therapy and Supportive Services
Connection to individual, family, or group therapy, psychological testing, community resources, and other supportive services when additional care can enhance treatment and recovery.

Do I need a referral?
Many patients can schedule an appointment without a referral. However, some insurance plans require one. We recommend checking with your insurance provider or contacting our clinic if you have questions.
What conditions do you treat?
Our providers diagnose and treat many mental health conditions, including:
- Anxiety
- ADHD
- Bipolar disorder
- PTSD and trauma-related disorders
- Obsessive-compulsive disorder (OCD)

What services do you offer?
Our outpatient clinic offers psychiatric evaluations, medication management and therapy services. Therapy may be available through our clinical social workers and therapists. Services are based on each patient’s individual needs and provider availability.
Do you offer Telehealth appointments?
Yes. Telehealth appointments may be available for established patients after an initial in-person evaluation. Availability depends on your provider, your treatment needs, clinical appropriateness and applicable regulations. Not all providers offer telehealth services.
How do I schedule an appointment, and what should I bring to my first visit?
To schedule an appointment, call our outpatient clinic at (605) 755-7200.
For your first appointment, please bring:
- A photo ID
- Your insurance card
- A current medication list
- Relevant medical records, if available
- Completed intake paperwork, if requested
What insurance plans do you accept?
We accept many commercial insurance plans, Medicare, Medicaid and other insurance programs. Coverage varies by plan and provider.
If you do not have insurance, self-pay options are available. Financial assistance may also be available through the Monument Health Financial Assistance Program for eligible patients. Please contact our office to verify your insurance benefits or discuss payment options before your appointment.
How do I request a medication refill or manage my appointments?
The fastest way to request medication refills, send messages to your care team, or cancel or reschedule eligible appointments is through MyChart, Monument Health’s secure patient portal.
If you do not have access to MyChart, please contact our clinic for assistance. We recommend requesting medication refills before you run out of medication.
Can your providers complete school forms, 504 Plans or IEP documentation?
Providers may complete certain school forms or provide documentation, diagnoses and treatment recommendations when medically appropriate. However, eligibility for a 504 Plan or an Individualized Education Program (IEP) is determined by your child’s school and educational team. Our providers can collaborate with schools, when appropriate and with your consent, to support your child’s care.
What if I need to cancel or reschedule my appointment?
If you need to cancel or reschedule your appointment, please contact our office at least 24 hours in advance whenever possible. Established patients may also manage eligible appointments through MyChart. Providing advance notice allows us to offer appointments to other patients waiting for care.
