The MyChart patient portal gives you convenient and secure online access to your health care record. Whether you’re at work, on the road or at home, view your test results, message your health care team, manage appointments and more.

Happy Together

Happy Together gives you the ability to link your health records (i.e., medications, allergies, test results and appointments) from outside providers with your MyChart Powered By Monument Health account. Learn more in the FAQ.

Information Blocking

As part of new federal regulations, Monument Health will be sharing more notes and results starting Feb. 10, 2021. Please note that final test results (with rare exception) will be available to you immediately in MyChart. At Monument Health, we believe that sharing information builds trust and better relationships. We believe that you should be able to see your results as soon as they are available. However, this means that you may see results even before your health care provider has seen them. Some test results may be hard to understand. Other results may show a serious disease, like cancer. Please be assured we review every result and will contact you with any result that is concerning. You may always follow back up with your provider with questions or concerns.

Text Messaging

Monument Health may occasionally use text messages in order to communicate information to MyChart users regarding their account. Users can opt out of these communications at any time in MyChart, or by replying “stop” to the text messages. Message and data rates may apply from your cell phone provider.

All Monument Health hospitals, clinics, urgent cares and specialty treatment centers use the same electronic health record system, Epic, and offer MyChart to patients. MyChart is also available to patients of Dakota Radiology, Fall River Health Services, Philip Health Services, Same Day Surgery Center and Weston County Health Services, through a partnership with Monument Health.

This means that no matter which of these locations you visit, your health information will be accessible to your care team and will be accessible to you in MyChart. You only need one MyChart account even though you may be seen at multiple locations.
