Job Summary

The Ambulatory Case Manager (ACM) supports patient care coordination in the primary care ambulatory clinic setting and/or in conjunction with the Managed Care Contracting identified goals. The ACM primary responsibility provides clinical evidence-based care management support to assist the delivery of effective education, self-management and timely health care delivery to high risk and rising risk patients associated with the assigned ambulatory clinic and/or managed care attributed patient population. The ACM works collaboratively with the multidisciplinary care team to deliver seamless transitions through the continuum of care by guiding the patient through the healthcare system in an efficient and fiscally responsible manner to achieve desired outcomes. The ACM facilitates alliance between the health care team, patients, family/caregivers and community by assessing, planning, and advocating for health care needs of an individual and/or identified population for quality, cost-effective outcomes. In collaboration with identified program and system leadership, the ACM will be responsible for identified data collection, management and analysis to assess the impact of the program. The registered nurse will demonstrate performance consistent with nursing and case management professional standards of practice, care, and the Nurse Practice Act.

Job Description

Essential Functions:

  • Optimize care coordination with hospital, ED, consulting physicians, and community resources as necessary.

  • Collaborates with physicians, providers, and practice caregivers to identify appropriate patients for care management.

     Utilize evidence-based materials related to coordination of care and health transitions to facilitate identification of appropriate interventions for improvement and maintenance of health.

  • Maintains a working knowledge and relationship with community resources and payer benefits that link the individual with the most appropriate resources. Assists by maintaining expertise on benefits, reimbursement and contract/regulation changes per payer, managed care contract guidelines, Medicare and Medicaid to facilitate appropriate reimbursement, education and guidance to assist the Healthcare team and patient/family in decision making.
  • Collaborate with the interprofessional health team across health care settings to effectively implement coordination plans while maintaining fiscal accountability and individual patient advocacy.
  • Collaborate with the patient and multidisciplinary team in evaluating change readiness, assessing patient needs, developing plan of care, designing teaching plans/programs, and evaluating the patient’s progress toward health care goals and attaining expected outcomes. 
  • Educates the patient and the patient’s family/caregiver about self-management tasks they can undertake to gain greater control of their health status through patient involvement in activities to improve their health, encourage wellness, avoid preventable hospital admissions, support social determinants of health disparities and address advance care planning where applicable. Identifies potential barriers to learning and/or care delivery and makes appropriate suggestions for change. 
  • Responsible for the collaborative development and monitoring of identified outcomes measures.
  • Conducts current and retrospective clinical reviews for the purposes of determining best practice patient care outcomes including most cost effective, appropriate care for the patient, and the effectiveness of care coordination. 
  • Anticipates the needs the identified patient population, seeing that necessary documentation and pre-visit planning is completed or requested before patient visit. Utilizes available resources efficiently and takes cost-effectiveness into account when assisting patients in scheduling medical services. 
  • Stays informed of the latest developments, trends and changes in healthcare; regularly attends continuing education and training opportunities in case management field. 
  • All other duties as assigned.

Education – Completion of a nursing education program that is approved by a board of nursing

  • Collects and prepares biological specimens for analyses using appropriate techniques and maintains the integrity of the specimen in relation to the test being performed.
  • Determines acceptability of specimens within established guidelines, avoiding hemolysis, QNS, and wrong anticoagulant use.

Job Summary

The Radiology Technologist may work in ambulatory or acute care environments. Performs and is competent in Radiology under the direction of the Radiologist. The Technologist must be able to independently acquire the proper exam for the Radiologist’s interpretation. The Technologist is able to operate all radiological equipment in the proper setting. The Technologist is required to work with patients in age group specific environments: pediatric, geriatric, febrile, and acute care; adjusting technique and positions for optimal examinations.

The Technologist is responsible for exam room presentation, patient transportation, entering exams into the Radiology Information System, scheduling radiology exams, educating patients on exam prep instructions, obtaining correct orders and paperwork for records, billing, and communicating with Medical Staff as requested. Job Description

Essential Functions:

  • Coordinates the activities of the Division to accomplish the work in an efficient and effective manner.
  • Counsels Division personnel, as necessary and in a timely manner, for failure to comply with established policies and procedures; completes the disciplinary process with appropriate documentation.
  • Assures adequate staffing within departments supervised by analyzing needs of each dept. based on provider needs, employee needs, patient volume and emergency situations; staffing assignments must maintain the quality care standards, yet be cost effective for the organization.
  • Responsible for interviewing, hiring, orienting, evaluating, counseling, disciplining and terminating employees within departments of supervision to ensure that departments are delivering excellent Healthcare.
  • Assists the Manager in preparing the annual budget; identifies supply, personnel, equipment, and facility needs; presents justification in support of requests for additional personnel and equipment.
  • Attends and participates in scheduled Supervisors' Meetings to keep current on intra- and inter-departmental concerns, problems, and solutions. Attends departmental and hospital meetings and in-services.
  • Conducts Division meetings, at least on a monthly basis, to keep personnel informed of and involved in Division, Department, and Hospital matters.
  • Demonstrates a consistent familiarity with, an understanding and acceptance of, and abidance with the Department's and the Hospital's policies and procedures. Participates in policy development and revision when needed.
  • Consistently and positively works as a team member, helps coordinate the schedule with all staff members in the department to efficiently finish the work load. Is accountable at work and completes the necessary training to safely work and troubleshoot technical equipment.
  • Ensures the procedure and related supplies are billed correctly. Works with vendors to maintain all equipment used is safe and performing at acceptable levels.
  • All other duties as assigned.

Additional Requirements

Required:

Education – High School Diploma/GED Equivalent: General Studies

Graduate – Medical Imaging Program for Radiologic Technology or other modality

Experience – 3 + years of Medical Imaging Technologists Experience

Certification – Basic Life Support (BLS) Healthcare Provider Certification – American Heart Association (AHA) – Within 60 days of hire or transfer; ARRT Certificate – American Registry of Radiologic Technologists (ARRT); or American Registry of Diagnostic Sonographer (ARDMS)

Job Description

Essential Functions:

  • Oversees all functions of the clinic including but not limited to scheduling, registration, referral management and patient access, revenue cycle management, provider recruitment, imaging, laboratory, and clinical services.  Serves as a liaison to corporate service departments that provide and/or support these services.
  • Oversees and ensures efficient clinic operations, ensuring a high level of patient care, quality, service, and productivity.  Demonstrates strong knowledge of care delivery systems and implements evidence-based best practices.  Provides operational and technical direction.  Continually monitors the functioning of the clinic(s), resolves problems, and revises operations as needed.  Monitors key performance indicators and identifies and implements areas for improvement.
  • Supports the organizations purpose, vision, values, and strategic priorities.  Develops, implements, and oversees processes to ensure that established policies, goals, and objectives are achieved.
  • Actively supports, maintains, and promotes organization quality initiatives.  Communicates and educates physicians and caregivers as necessary to ensure organizational understanding and compliance with quality standards.
  • Responsible for the overall patient experience and resolves any patient concerns in a timely manner.
  • Establishes and maintains effective communication and develops professional relationships with physicians and advanced practice providers.  Responsible for provider relations including practice management, orientation and onboarding, and proactive problem resolution.
  • Develops, manages, and is accountable for annual operating and capital budgets.  Controls expenditures within approved budget.
  • Performs human resource responsibilities including interviewing and selection of new caregivers, coaching and development, performance appraisals, conflict resolution, corrective actions and terminations.
  • Provides exceptional leadership and promotes teamwork and collaboration.  Is accountable for the overall engagement of physicians and caregivers by creating an environment in which people feel valued and respected.
  • Plans and facilitates regularly scheduled provider and caregiver meetings.
  • Ensures compliance for various accrediting bodies and governmental agencies and performs all necessary reviews, audits, education, and training to maintain compliant status. 
  • Determines physical maintenance, facility, and equipment needs and repairs and coordinates with internal and external resources. 
  • Participates in community activities and builds and maintains positive community partnerships/relationships.
  • Performs work on projects, special assignments, committees, and other duties as assigned or as necessity dictates.
  • All other duties as assigned.

Additional Requirements

Required:
Bachelor’s degree (or equivalent knowledge) in business administration, health care administration, nursing, or other related field.

Preferred:

Certifications – Registered Nurse (RN) – South Dakota Board of Nursing

Typically requires 3+ years of experience in health care setting including knowledge in medical group practice management and ambulatory clinic operations.  Includes at least one year or more of supervisory experience in managing staff.
