Job Description

Essential Functions:

  • Assists in discharge plan development, giving support to discharge planner as needed.

  • Completes hospital-wide mandatory in-services as well as age-specific unit education requirements (pediatrics, adolescents, adults, and geriatrics).

  • Completes Suicide Risk assessment within predetermined guidelines. Recommendations are integrated into the treatment plan.

  • Conducts and coordinates mental health and Suicide risk assessments, consults for at-risk patients, and/or screenings in the Emergency Department, nursing towers, or Behavioral Health Center to make appropriate referral recommendations to inpatient and outpatient providers consistent with the needs of the patient.

  • Conducts appropriate assessment, utilizing pertinent data, develops an initial treatment plan, communicating this to the charge nurse and assigned staff member upon admission to the appropriate floor.

  • Coordinates admissions to the Behavioral Health Center through the Emergency Department, nursing towers, and physicians.

  • Coordinates the appropriate level of care with the admitting or on-call physician – communicating to the charge nurse the patient status.

  • Develops and maintains positive partnerships with providers of mental health and social services in the community as appropriate to the needs and requirements of the patient on an ongoing basis.

Required:

Education – Bachelor’s Degree- Human Services, Psychology, Public Health or other similar/relevant degree required

Job Description

Essential Functions:

  • Performs clinical duties under the direction of a licensed Respiratory Therapist.
  • Demonstrates the ability to identify an emergency situation and initiates appropriate responses.
  • Demonstrates knowledge of technical principles of cleaning, standard precautions (BSI), assembly, and testing of respiratory and life support equipment.
  • Performs technical error trouble shooting of all respiratory care equipment and reports safety issues/faulty equipment to appropriate personnel or department for repair.
  • Monitors, organizes, and replenishes respiratory care supplies/equipment in the department and designated areas to include cost effective ordering of stock and non-stock supplies.
  • Investigates and resolves delayed deliveries, backorders, and other discrepancies; reports progress and outcome to Director.
  • Maintains an adequate supply of oxygen/air/nitric oxide/heliox cylinders in designated areas by checking pressure levels and replacing cylinders as needed. Inspects storage areas and assures proper storage of cylinders per safety standards.
  • Performs all projects and assignments delegated by Director/Supervisor.
  • Answers the telephone efficiently and effectively, transferring telephone calls appropriately.
  • Communicates with patients to ensure scheduled exam times are acceptable, and requirements for exams are understood. Documents changes in scheduled exams and patient demographics in the appropriate systems. Ensures all orders placed in the system meet the time frame requested by the provider ordering the exam.

    Greets patients and visitors in a pleasant manner, effectively handling their needs.

  • Assesses patient’s progress and modifies treatment plan accordingly.
  • Develops and maintains positive partnerships with providers of mental health and social services in the community as appropriate to the needs and requirements of the patient on an ongoing basis.
  • Maintains good communication and collaborative relationships with referring providers and other staff. Recommends additional referrals for patient as needed.
  • Functions as a positive resource, communication coordinator, and role model for departmental staff. Maintains accessibility and visibility within the department.
  • Assists patients to maintain their sense of competence, identity and autonomy.
  • Demonstrates effective collaboration with community/regional services and hospital personnel while maintaining patient and family rights.
  • Assists in discharge plan development, giving direction to discharge planner as needed.
  • Completes discharge planning note/assessment within predetermined guidelines. Upon completion of assessment, recommendations are integrated into the treatment plan 95% of the time.
  • All other duties as assigned.

Additional Requirements

Required:

Education – Masters degree in Counseling or Social Work
Certifications – Licensed Professional Counselor (LPC) – South Dakota Department of Social Services or Certified Social Work (CSW) – South Dakota Department of Social Services

Job Description

Essential Functions:

  • Provides direct and indirect supervision of psychometrist.
  • Maintains appropriate records in accordance with state, federal laws and Monument Health policies, procedures and standards.
  • Performs evaluation/testing to determine the most effective approach to care and to clarify diagnosis.
  • Conducts Healthcare teaching and counseling of patients/families regarding health problems and management of illness.
  • Demonstrates the ability to appropriately assess patients, apply clinical interventions, evaluate outcomes, and implement changes to the plan of care. Anticipates and communicates changes in patient status to meet the needs of the patient.
  • Documents all procedures and/or services to patients in accordance with established procedures to ensure accurate billing to patient accounts.
  • Performs and documents comprehensive initial patient assessment, utilizing age-specific growth and development parameters.
  • Performs and documents ongoing patient assessment as warranted and formulates plan of care in collaboration with other medical providers.
  • Recognizes physical, developmental, and psychosocial distress of patient and family, and provides appropriate interventions.
  • Provides cross-coverage for other providers and exhibits an ability to adjust to other providers’ preferred style.
  • Completes documentation according to facility policy.

Required:
Education – Doctoral degree from an APA accredited program.  An APA accredited internship
