You do NOT need to print or transfer your medical records if you plan to continue your care with Monument Health.
When you schedule and receive care at any Monument Health location, your complete medical record will be transferred into Monument Health’s electronic health record. This allows your care team to have the information they need when Monument Health Gillette Clinic opens. Please check back for more information on scheduling in the future. Gillette Clinic is a Department of Monument Health Rapid City Hospital.
WHAT DO I NEED TO DO WITH MY MEDICAL RECORDS?
If you are not planning on continuing your care with us or would like a copy of your records:
Through August 31, 2026:
- Download your records through the Athena Health patient portal, or
- Request your records by calling (307) 387-9850.
After August 31, 2026:
All Hoskinson Health & Wellness medical records requests should be directed to Cariend, the medical records custodian, or by calling (855) 516-0612.

If you would like a copy of your medical records for your own records, download them through the Athena Health patient portal before August 31.
SEPTEMBER 1, 2026
Medical records requests transition to Cariend.
After August 31, all Hoskinson Health & Wellness medical records requests should be directed to the medical records custodian, Cariend.
OCTOBER 2026
Watch for more information regarding Physicians and Services.
Patients will receive a letter with the complete list of providers and services joining Monument Health Gillette Clinic.

If you plan to continue your care with Monument Health, no. Your complete medical record will be transferred into Monument Health’s electronic health record when you schedule and receive care at a Monument Health location.
What if I want a copy of my medical records?
You can download them through the Athena Health patient portal before August 31, 2026, or request them by calling (307) 387-9850.
What happens after August 31?
Medical records requests should be directed to Cariend ROI at (855) 516-0612 or through the Cariend Medical Records Request Portal.
Will I receive more information?
Yes. In October, patients will receive a letter with the complete list of providers joining Monument Health Gillette Clinic and additional transition information.

Will the current Gillette Clinic location at 620 Four-J Court remain the same?
Yes, Monument Health will continue to operate the clinic at 620 Four-J Court. Eventually, the services will be moved to the new location. Patients will receive a letter indicating when that move will occur.
