About The Program

The Cancer Transitions: Moving Beyond Treatment program is designed to improve quality of life by focusing on lifestyle skills and habits that matter most to cancer survivors.

Through this program, participants meet for one 90 minute session each week, for four consecutive weeks. Education, group discussion, video modules of expert speakers, and an emphasis on setting and achieving goals ensure that participants have a unique and enriching experience.

Led by a cancer survivor and passionate advocate committed to helping survivors find their strength, this program provides an opportunity to learn from one another and feel a sense of community. This is a special opportunity to connect with other people with shared experiences to talk about issues and concerns.

Our comprehensive approach addresses the whole person, focusing on six critical areas of survivorship:

Eating & Nutrition

Learn healthy eating habits and nutrition strategies to support your recovery and long-term wellness.

Emotional Well-being

Develop tools to manage stress, anxiety, and emotional challenges that come with life after cancer.

Financial Wellness

Navigate the financial aspects of cancer survivorship with practical resources and guidance.

Medical Care

Understand your ongoing medical needs and learn to effectively communicate with your healthcare team.

Physical Activity

Discover safe and effective ways to stay active and rebuild your strength after treatment.

Relationships

Strengthen connections with family and friends while building new supportive relationships.
