Attending your Pre-Employment Health Screening is a vital step prior to starting your career with Monument Health. Here are the required documents to bring with you. You will have a health screen, blood draw, and drug test. Remember to dress to take a photo for your Caregiver ID Badge.
- I-9 Verification Documents
- Immunization Records — If you received your vaccinations in South Dakota please use this link to request a copy.
