LPNs working in ambulatory settings offer both direct and indirect patient care, working together with other healthcare professionals. They provide nursing care to specific patients under the supervision of a physician or Registered Nurse (RN) and undertake delegated responsibilities to meet patient requirements. LPNs deliver nursing care in alignment with Monument Health's policies and procedures, professional standards, and in compliance with the South Dakota Nurse Practice Act, alongside other relevant federal, state, and local regulations.

Job Description

Essential Functions:

  • Sets up the examination and treatment areas appropriately, in line with clinic requirements.
  • Ensures proper patient care by actively participating in pre-visit planning and arranging exam and treatment spaces according to clinic policies.
  • Conducts patient rooming procedures by monitoring vital signs, collecting data, and attending to other clinic-related tasks.
  • Proficiently follows clinic policies and best practices when administering and documenting medications.
  • Administers nursing interventions and treatments as per the Scope of Responsibilities, complying with the nursing division and clinic's particular standards.
  • Illustrates proficiency in conducting precise assessments, administering clinical interventions, evaluating outcomes, and adjusting the care plan collaboratively with an RN or healthcare practitioner.
  • Provide pre-authorized patient education pamphlets. Support the health teaching and discharge guidance administered by the RN or healthcare provider, guiding patients to seek further information from the RN or provider for any clarifications needed.
  • Executes solo nurse visits as permitted by department policy and within the appropriate level of supervision.
  • Assist in inbox administration and the fulfillment of patient referrals and authorizations within the guidelines and allowed scope of practice.
  • Plays a role in aiding and guiding clinical staff during their onboarding process under specific directives.
  • Shows support for departmental progress by participating in meetings, committees, and continuous education/training programs.
  • Makes certain that documentation in the patient record is accurate and succinct within the defined Scope of Responsibilities.
  • Additional assignments as deemed necessary.

Additional Requirements

Required:

Education - Completion of a nursing education program that is approved by a board of nursing

Certifications - Licensed Practical Nurse (LPN) - South Dakota Board of Nursing; Basic Life Support (BLS) Certification - American Heart Association (AHA) - Within 60 days of hire or transfer.

Regarding BLS certification:

The department you are applying to may require BLS certification sooner than 60 days post transfer/hire. The hiring manager will discuss this with you if it is applicable.

Preferred:

Work Experience - 1+ years Healthcare Experience; 1+ years Licensed Practical Nurse Experience

Physical Requirements:

Very Heavy work - exerting over 25 pounds of force constantly (67-100% of the time), and/or more than 50 pounds frequently (34-66% of the time), and/or more than 100 pounds of force occassionally (up to 33% of the time), and/or more than 100 pounds of force seldomly to move objects. Possible exposure to radiation and frequent exposure to contaminated needles and infectious body substances. Requires the ability to work designated shift lengths (including 8 and 12 hours), which may include night shifts or occasional irregular hours.
