Job Summary

Coordinates, plans and delivers the best psychotherapeutic approach for each patient. Provides clinical (only) supervision for day-to-day implementation of therapeutic strategies or milieu programming that carries out the treatment plan for each patient. Assists the dialysis patient in adjusting to a life on dialysis therapy. Provides resources to meet the needs of the dialysis patient.

Job Description

Essential Functions:

  • Contributes to unit/department development as evidenced by attendance or participation in department meetings.
  • Advocates for the patient as well as the payer to facilitate a positive outcome for the patient, Healthcare team, and payer. Makes referral to Midland Professionals on all South Dakota residents that have no third party coverage.
  • Assessment, Crisis Intervention, Short- term Counseling and Referral.
  • Develop and maintain positive relationships with all internal and external management personnel, providers of mental health and social services in the community as appropriate to the needs and requirements of the applicable customers on an ongoing basis.
  • Displays a professional appearance in dress, mannerisms, and team interactions. Displays and/or supports the team concept with behaviors that reflect understanding of the Center’s interdependent needs.
  • Assists with patient/family teaching at an age-appropriate level, as evidenced by chart review/observation.
  • Completes social assessment within predetermined guidelines. Recommendations are integrated into the treatment plan.
  • Conducts initial and care plan reviews according to unit policies. Observed by chart review. Reviews and updates target dates on care plan.
  • Functions as a positive resource, communication coordinator and role model for departmental staff, maintaining accessibility and visibility within the department.
  • Collaborates with patient/family, multidisciplinary team, and physician to formulate a realistic plan that identifies goals, specific interventions, and resources to meet the patient's needs.
  • All other duties as assigned.

Additional Requirements

Education - Masters degree in Social Sciences
Certification - Certified Social Work (CSW) - South Dakota Department of Social Services

Physical Requirements:
Sedentary work - Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time.
