Job Summary
Responsible for the oversight, reconciliation process, and maintenance of the practice census of patients assigned to Hospitalist Services. The position will ensure patient data integrity by working closely with a variety of internal departments such as but not limited to medical staff, admissions department, emergency room department health unit clerks, nursing staff, patient financial services, and ancillary staff. Conducts new provider orientation for the Census Coordinator role, schedule, policies, and patient assignment. This position will ensure and standardize the work assignment through the implementation of process changes that come out of the department’s needs and general ongoing process improvements to patient/provider assignment. Maintain scheduling updates and changes to providers’ schedules as assigned or as director by the Medical Director or Department Manager to assure coverage for all provider shifts. The position will consistently use verbal, nonverbal and written communication. Job Description
Essential Functions:
- Assists leadership with the development, coordination and maintenance of schedules for the department. Provides collaborative support to meet daily staffing requirements and maintains effective staffing patterns.
- Ensures staffing needs are communicated with all staff as required. Authority to determine coverage needs for provider schedule changes to assure adequate coverage and make changes in the absence of leadership.
- Physician worked shift reconciliation and tracking – assists department manager with the monthly tracking and quarterly and annual reconciliation of actual shifts worked per provider.
- Ensure that the implementation of all department patient flow and process changes are made effectively and timely. Backup Census Coordinator when coverage is necessary, i.e. must demonstrate the knowledge and skill set to function as a Census Coordinator and to be a resource to the department and facility as it relates to the Census Coordinator position.
- Effectively communicates with all staff and works collaboratively to ensure processes are in place for accurate patient counts with the assignment from all sources and patient referrals.
- Attend facility meetings that impact patient flow for the department and the Census Coordinators’ impact on the patient flow for the facility as required. In emergent situations, be able to prioritize the assignment of patients and oversee the flow of patients through the Census Coordinators’ process.
- Participates in the department orientation to assure training to new staff on the processes and procedures of the Hospitalist Census Coordinator role and patient assignment.
- Communicate with health care professionals and others as necessary, obtaining information, analyzing situations and effectively responding to achieve cooperative satisfaction.
- Complete these communications immediate where necessary or in a timely manner. Immediately inform Medical Director or Department Manager of troublesome or other communication problems with others.
- All other duties as assigned.
