Job SummaryThe Patient Navigator assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal outcomes for patients. The Patient Navigator consistently employs patient and family-centered care to promote safety, quality, and satisfaction.
●Analyzes patient diagnosis and relates information to physician and clinical team.
●Builds awareness in the patient of all available services, assists in clarifying choices and ensures unrestricted access to all services.
●Contacts all patients referred to introduce self and program to understand needs (family support, socioeconomic status, etc.) and answer questions/concerns.
●Coordinates patient information with community physicians and customizes patient schedules.
●Initiates meetings and/or makes arrangements with support departments to meet the needs of patients; finance, counseling, social services/child issues, etc.
●Initiates meetings/communications with physician offices or related professional personnel to build or maintain communication to increase awareness and/or address processes for the patient navigator program, Organizes and helps create plan customized for each patient referred; scheduling, supportive resources (i.e. counseling, dietician, wig services), and post-treatment resources (i.e. support groups).
●Participates with multidisciplinary team meetings discussing future and present patient cases and collaborates efforts to meet needs identified.
●The Navigator coordinates data, community physicians, and Regional Health resources into a seamless model of access and care benefiting patients, participating physicians, and family members.
●Develops and promotes standardized training and documentation in the applicable field
●Assists with developing health protocols and clinical practice guidelines.
●Provides data, facilitation, and consultation as required.
●All other duties as assigned.
Education - High School Diploma/GED Equivalent in General Studies
Experience - 1+ years of Healthcare Experience
Education - Bachelors degree in Related Field
Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.10 Monument Health Rapid City Hospital, Inc.
