Job Summary

The Health Coach is an integral part of the patient care team. They have an active role in planning, facilitating and coordinating patient lifestyle modifications at both the individual and group level, particularly those patients with chronic health conditions. The Health Coach is 100% patient centered, utilizing a progressive, collaborative approach to optimize patient care and outcomes.

Job Description

  • Collaborates with a wellness team to plan the best course of action to support patients in successfully achieving their goals.
  • Performs initial patient assessment interviews, applying motivational interviewing and engagement techniques.  Communicates accurate and timely information regarding the patient's condition to the appropriate caregiver or provider.

  • Actively participates with patients in individual and group visits and wellness classes and activities, anticipating and addressing wellness needs through coaching techniques.

  • Identifies the need for behavioral change to improve health status, reduce health risks and improve quality of life.

  • Monitors and evaluates the progress of the patient.  Assess patient progress toward care plan and self-management goals.  Evaluates the effectiveness of the plan in meeting established care goals; revised the plan as needed to reflect changing needs, issues and goals. 

  • Routine and timely documentation of all core process work activities including, but not limited to, chart reviews, patient calls and correspondence and patient education activities.

  • Identifies and effectively facilitates the utilization of community resources to meet the needs of patients based on their established goals and plan of care.  Facilitates patient access to community resources as appropriate.

  • Maintains awareness and actively participates in Regional Health's Purpose, Vision, Values and Priorities. Meets expectations and maintains competencies of new hire orientation checklist, annual competency verification, and participates in education/quality improvement programs as directed.

Additional Requirements


Education: Associates (Healthcare Related Field)

Certification - Health Coach Certification - National Society of Health Coaches (NSHC) (Certification to be obtained within one year of employment)
