Job Summary

Monument Health is seeking a full-time APP to work at an established practice in Rapid City, SD. The primary responsibility for this role will be to evaluate, manage, and treat cardiac surgery patients under the direction of the Cardiac Surgery attending physician in the inpatient setting. The secondary responsibility for this role will be assisting with patient care in the clinic setting. This role will have five scheduled shifts per week and may participate in weekend inpatient coverage as part of a rotation with other members of the cardiac surgery team. We are looking for someone motivated and driven, with outstanding patient care and communication skills.

Job Description

  • Evaluates and treats post cardiac surgery patients. Manages acute post op issues as well as chronic co-morbidities.

  • Works closely with multi-disciplinary team to coordinate patient throughput and discharge planning.

  • Obtains complete histories and performs pertinent physical exams with assessment of normal and abnormal findings.

  • Performs or requests and evaluates diagnostic studies

  • Administers, orders, furnishes and/or prescribes medications

  • Orders and collects specimens for routine laboratory tests, screening procedures and therapeutic procedures, including blood and blood products

  • Orders physical therapy, occupational therapy, respiratory therapy, radiology examinations and nursing services

  • Effectively communicates and interacts with patients, families, staff and members of the community from diverse backgrounds

  • Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary

  • Facilitates the coordination of inpatient and outpatient care and services

  • Facilitates collaboration between providers and coordination of community resources

  • Provides and coordinates patient teaching and counseling.

  • Tracks and interprets test results and answers questions either directly or over the phone

  • Develops and implements processes / procedures to promote effective and efficient use of resources.

  • Other duties as required.

Additional Requirements

Required:
Certifications - Certified Nurse Practitioner (CNP) - South Dakota Board of Nursing or Physician Assistant (PA) - South Dakota Board of Medical and Osteopathic Examiners

Preferred:

Experience - 1+ years of Clinical Experience; 1+ years of Healthcare Experience

Physical Requirements:
Light work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
