Job Description

Essential Functions:

  • Screens all calls and either routes them to their destination or takes appropriate action to fulfill the request of the caller. Ensuring that accurate and timely communication is facilitated and that the department is presented in a positive manner.
  • Knowledgeable of all emergency procedures outlined in the EPM. Implements proper action in drills and actual disaster or emergency situations. Files incident report following activation of these procedures. Reads the EPM and initials quarterly.
  • Is aware of security procedures to: ensure appropriate physical access, system/network access, and process/data integrity. Identifies areas where existing security policies and procedures require change or where new ones need to be developed.
  • Employee is proficient with all computer applications required and used in the Communications Center environment and is able to effectively utilize the applications on a daily basis, as well as for special projects upon request.
  • Employee is knowledgeable of all departmental equipment including alarm panels, hearing impaired/language line phones, and Zetron computer. Has the ability to understand equipment readouts, relay pertinent information, and perform other routine tasks.
  • Knowledgeable and can perform the intake of repaired, returned, new or loaner pagers from vendor and properly documents, copies and processes all required steps to the process including notification to using departments.
  • Coordinates courier or other various services as necessary in a professional and courteous manner. Ensures proper documentations of request and action is taken.
  • Is flexible in work and break scheduling to optimize departmental support to the organization. As needed, responds to after normal working hours; to monitor, respond to, and remedy problems or emergency situations that may occur.
  • Provide support to the organization, division, and peers as a critical function of all IT professionals. Support includes day-to-day peer level support, emergent after hours support, and any support required to maintain highly reliable IT systems.
  • Document processes and activities appropriately to maintain highly reliable IT systems. Provide required elements to Call Tracking, Project Management, Change Management, and Disaster Recovery Services Guides to achieve this goal.

Job Description

Essential Functions:

  • Conduct orthopedic educational programs and monitor effectiveness of such programs. Facilitate family involvement by supporting the “coach” role in the joint replacement process.
  • Consistently keeps patient flow running smoothly. Demonstrates ability to make patient care decision based on the urgency of individual needs. Coordinate all patient processes and needs from the initial pre-op testing and patient registration to post op care. Serve as a liaison in the coordination of the patients preoperative care between physician's offices, the hospital and other facilities.
  • Develop, implement and monitor the compliance with multidisciplinary orthopedic care pathways and revise based on patient satisfaction, standards of care and quality information.
  • Anticipates the needs and wants of the provider by preparing the patient for surgery.
  • Assess, set goals; prepares, implements, documents and adjusts the plan of treatment and education for each patient based on the patient's individual circumstances, medical condition and age, to meet the expectations of the patient, physician, and program.
  • Market the orthopedic program through outreach community education, utilize “Graduates” as ambassadors, and provide evidence based patient feedback to the orthopedic surgeons and providers.
  • Monitor joint replacement clinical outcomes and identify opportunities for improvement. Monitor growth of program as well as fiscal impact to the organization.
  • Monitor patient satisfaction through patient satisfaction surveys and conduct in-house discharge phone calls to gather information regarding patient’s perceptions of care, patient outcomes and the joint replacement experience.
  • Organize and lead the Orthopedic Program and the Joint Center of Excellence. Collaborate and coordinate with all disciplines regarding patient care. Receive information from multidisciplinary joint care team and drive solutions for identified problems.
  • Consistently sets an example of excellence in clinical practice and professional behavior. Acts as a resource to clinical staff, patients, families and other customers regarding patient care. Mentors employees in the development and enhancement of clinical skills.

  • Supports compliance with Utilization Review and regulatory guidelines. Identify trends related to compliance of regulations and collaborates with appropriate hospital personnel and external agencies to ensure appropriate process changes are implemented.
  • Supports the Medical Staff Quality Review process by identification of issues related to quality indicators set forth by the Medical Staff.
  • Creates and updates the Plan of Care (POC) to include patient/family-centered goals with interventions that reflect the changing needs of the patient/family unit and is consistent with their current needs and desires.
  • Maintains a working knowledge and relationship with community resources and payer benefits that link the individual with the most appropriate resources. Assists by maintaining expertise on benefits, reimbursement and contract/regulation changes per payer guidelines, Medicare and Medicaid to facilitate appropriate reimbursement, education and guidance to assist the healthcare team and patient/family in decision making.
  • Facilitates planning for patient/family needs to ensure a smooth transition for the patient across the continuum of care.
  • Evaluates current treatment plan to identify barriers, determine realistic goals and objectives, and seek potential alternatives in conjunction with the medical staff. Coordinates team efforts with support services departments to ensure appropriate care and smooth discharge transition.
  • Is a visible resource on the unit and coordinates interdisciplinary team huddles/communication, identifying and communicating the patient’s health care needs based on the best practice standards to ensure care and communication needs are met in relation to both internal and external providers/services, ancillary department services, core measures, compliance with internal policies/regulatory guidelines and True North metrics.
  • All other duties as assigned.

Education – Completion of a nursing education program that is approved by a board of nursing

Certification – Registered Nurse (RN) – South Dakota Board of Nursing

Certification – Commission for Case Management Certification (CCMC) – Accredited University or accredited training professionals
