Job Summary

The Licensed Practical Nurse (LPN) works as part of a team to deliver both direct and indirect patient care in an outpatient setting. Under the supervision of a physician or Registered Nurse (RN), the LPN provides care to assigned patients and performs delegated tasks to address their needs. Care is provided in alignment with Monument Health’s policies, professional standards, the South Dakota Nurse Practice Act, and relevant federal, state, and local regulations.

Job Description

Essential Functions:

  • Establishes the examination and treatment spaces as per the clinic's guidelines.
  • Participating in pre-visit coordination and arranging exam and treatment spaces according to clinic standards is essential for adequately caring for patients.
  • Carries out patient rooming processes, incorporating vital signs recording, data collection, and other essential aspects tailored to the clinic's operational standards.
  • Effectively implements medication administration and documentation in adherence to clinic policies and recognized standards.
  • Undertakes nursing procedures and treatments guided by the Scope of Responsibilities identified by the nursing division and clinic's standards.
  • Shows mastery in performing and documenting accurate assessments, executing clinical interventions, assessing outcomes, and collaboratively adapting the care plan with the RN or provider.
  • Offer pre-existing patient education materials that are preapproved. Reiterate the health education and discharge instructions conveyed by the registered nurse or healthcare provider, ensuring that patients direct any questions to the medical professionals.
  • - Executes nursing visits independently following the permitted scope of practice and the department's policies to ensure the provision of suitable supervision.
  • Aid in the supervision of email organization and the successful processing of patient referrals and pre-authorizations as per the applicable rules and obligations.
  • Engages in the onboarding and mentoring of clinical staff in alignment with their scope of work and specific instructions.
  • Promotes departmental development by participating in meetings, committees, and continuing education or training activities.
  • Guarantees that information in the patient record is both clear and accurate and falls within the outlined Scope of Responsibilities.
  • All remaining tasks as given.

Additional Requirements

Required:

Education - Completion of a nursing education program that is approved by a board of nursingCertification - Licensed Practical Nurse (LPN) - South Dakota Board of Nursing

Preferred:

Experience - 1+ years of Healthcare Experience

Physical Requirements:

Very Heavy work - exerting over 25 pounds of force constantly (67-100% of the time), and/or more than 50 pounds frequently (34-66% of the time), and/or more than 100 pounds of force occassionally (up to 33% of the time), and/or more than 100 pounds of force seldomly to move objects. Possible exposure to radiation and frequent exposure to contaminated needles and infectious body substances. Requires the ability to work designated shift lengths (including 8 and 12 hours), which may include night shifts or occasional irregular hours.
